Use this form to request deletion of a desk phone line of service or a mobile phone.
Desk phones
When a desk phone line of service is discontinued, the physical desk phone will be picked up by a technician. You will need to provide the following information to complete the request:
- Number to be removed;
- Name of user assigned to the number to be removed;
- Location of device (building & room number);
- Budget number; and,
- Budget manager name.
Mobile phones
When a mobile phone line of service is discontinued, the mobile phone must have any personal accounts (iCloud, Google, Samsung, etc.) removed, be reset to factory defaults and turned in to the supervisor or Office of Technology. You will need to provide the following information to complete the request:
- Number to be removed;
- Name of user assigned to the mobile number;
- Budget number; and,
- Budget manager name.
