This service request should be used for contractors or vendors that are not employees of UCM and termed an external affiliate (XA). External affiliate accounts are for a period of time with a start and end date of access based on the contractual agreement or memorandum of understanding executed between University of Central Missouri and the contractor/vendor. Student workers are to be entered through the EPAF. Non-instructional affiliates should be requested through this form. For instructional external affiliates please contact Human Resources to process their account. If approved, this user will only receive a Network ID so they can log into the network while on campus.
If additional access is needed for this user, please list the accesses requested in the description and permissions will be considered and reviewed.
