Report An Incident

About

Use this form to report a security incident. Involving but not limited to: disclosures of protected information, system compromise, unauthorized access, violation of the Acceptable Use Policy, malware, loss or theft of hardware or compromised account.

DO NOT use this form to report threats to personal safety or criminal activity.  Contact Public Safety at (660) 543-4123 or 911 for emergencies 

Information

Helpful information to include in the description:

  • Brief description of the information security incident 
  • Date & time the incident was observed (approximations are fine)
  • Other sources that may have been impacted
  • Describe any actions you have taken in response to the incident 

Directions

  • Review the information to include above
  • Click "Report an Incident" on the right hand side
  • Fill out the information to the best of your ability. 
  • Click the "submit" button at the bottom
  • IF you chose 'yes' to be contacted - wait for further information from our Security team

If you suspect a system to be compromised, please take the following steps: 

  • IMMEDIATELY stop using the system. DO NOT power off the device. DO NOT use the keyboard
  • Disconnect the network cable or disconnect from WiFi
  • Report the incident and await further instruction 

Scope

This service applies exclusively to UCM data, UCM-owned devices, and any devices connected to the UCM secure network.
