Request a New Phone (Desk or Mobile phone)

Use this form to request a new line of service for a desk or mobile phone. Below is additional helpful information about desk and mobile phones. Do not use this request for an update to an existing desk or mobile phone. Choose "Change or modify a desk or mobile phone" for this service.

Desk phones

Use this form to request the installation of a new desk phone and line of service. When making this request, you will need the following items of information to complete the request:

  1. Primary user name and 700#;
  2. Contact for the request;
  3. Budget number affiliated with the department;*
  4. Budget manager name;
  5. Location (building and room number in which the phone will be placed);
  6. User's choice of standard voice mail or voice mail to email (voice mail is required on each phone unless otherwise requested by a Dean for faculty or Director for staff; and,
  7. Whether or not the line will be added to a hunt group or part of an auto attendant.

*The budget number is used for tracking purposes and is not necessarily for charging a department unless the department is a grant funded, auxiliary, contractor or third party.

What is single number reach?

Single number reach is a service in which users can choose to enable the ability to be reached via a single UCM office phone number that rings on both their office desk phone and their mobile phone, simultaneously. Mobile users can pick up an incoming call on either of their desk or mobile phones. Time of day scheduling can be applied to this service.

What is a hunt/line group?

A hunt/line group is a form of phone automation that allows calls to ring on multiple lines until someone is available to answer. If all users are busy and cannot take the call, it could then be routed to voice mail. What is an auto attendant?

An auto attendant is a system that allows incoming calls to be transferred to various extensions depending on the nature of the call and the callers selection. If this is a service your department desires, please select "Request This Now", complete the form and a technician will arrange a consultation with you.

Mobile Phones

If you are requesting a new mobile phone line of service, the following items of information must be provided:

  1. Supervisor approval for the mobile phone purchase;
  2. Name and NID of the user;
  3. Type of phone preferred (iPhone or Android);
  4. Budget number; and,
  5. Budget manager name.

It is highly recommended that the department purchase a mobile phone case and screen protector for the phone being purchased.
